You will be responsible for providing the highest quality care, at home, or out and about, including monitoring client’s care and well-being to deliver a quality, personalised and responsive care service for clients and carers. Client welfare and carer support needs will evolve and adaptability is required, as is a client focus and demonstration of the charity’s values and ethos.

Key Responsibilities:

Client and Carer Well-being, Support and Care.

Work as part of an award winning team to provide an excellent care service for older people and/or their carer in order to support independence. To promote Extra Years of Zest and enhance their quality of life and provide the support to do the things that are important to them.

General Responsibilities:
- Work with individual clients and carers assigned to you as their key worker and implement and review care plans.
- Ensure any changes or relevant information is captured fully and written in the client’s care plan and that visit reports and risk assessments are recorded accurately.
- Work to policies and procedures that govern the provision of care services to meet the Health and Social Care Standards, SSSC Codes of Practice and which comply with Health and Safety.
- Employ safe working practices.
- Work flexibly, to be redeployed when clients are in respite.
- Engage in opportunities for continuous professional development.
- Treat with confidentiality any private or sensitive information.

Key Requirements:
- Must be minimum SVQ Level 2 in Health and Social Care or working towards such a qualification or equivalent.
- PVG Scheme membership.
- SSSC Registration, when required.

Experience:
- Delivery of care services for people with dementia or older people with physical or mental frailty.
- Work in a customer focussed service, maintaining excellent quality standards.
