Would you like a fulfilling career in a local charity that places the greatest importance on the quality of the patient experience and the welfare and development of staff? To work within a strong multi-disciplinary team caring for adults who may have different needs?

If so, this hospice is looking for enthusiastic dedicated people to join their team to work as part of a new exciting pilot project providing care at home, in their catchment area. In this role you will be able to make a lasting difference to the lives of people through care that will have a positive impact on the community for many years to come.

You will work rostered shifts, 7 days a week between 8am to 10pm, with some care at night. You will need to be able to work independently and as part of a team. You will be able to demonstrate excellent listening and communication skills and be a car driver, with access to a car.

The Senior Community Support Workers must have previous experience in an equivalent role with the ability to lead and mentor others on shift. Shared values, discretion, respect and a passion for caring for others is essential for all their Support Workers. Practice development, clinical supervision and ongoing education is encouraged and supported by the Hospice.

Overall Job Purpose::
- To be friendly, open, trustworthy competent to give intimate personal care, practical and emotional support and to prompt concordance with prescribed medication either working alone or as part of a team
- Be able to build rapport quickly in order to gain the confidence and trust taking direction from the Senior Support Worker, Team Coordinator, the patient and family carers in order to assure dignified, safe personal care
- To assure continuity of care through effective handover ensuring that you have picked up the necessary information to deliver care in a patients home
- To observe and report changes in symptoms, condition and circumstances entering this information accurately in the care plan as required on shift
- To be responsive and flexible in meeting the needs of patients and families within the agreed care plan
- Update care notes in the patients home and update their electronic care records accurately- To be responsive and flexible in meeting the needs of patients and families within the agreed care plan
- To work on the Inpatient Unit (IPU) as and when required,

Please note the successful candidate will be required to undertake an enhanced level DBS check.
